Bring This To Your District.

This page is not for the six districts already in the cohort. Their records live under Districts & Schools. It is for a district, county office or consortium that wants to do this next: what the first cohort committed to, the instruments it used, how the work is funded and sustained, and where to start. Everything here is the published record, not a pitch.

Cohort I · 2026–2028·Six districts · eighteen schools · four counties·The Training Guide (D7) Will Formalize This In 2027–28

Who This Is For

Three Ways In.

A District On Its Own

Adopt the model with the county office or Impacter Pathway directly: three schools, a district lead, the readiness assessment, a fall screening window, a standing data review. The instruments below are published so a district can start without waiting for a cohort.

A Future Cohort

The Commission funded Cohort I to learn how a statewide performance management system is built. What is learned here is written down so Cohort II starts further along: recruitment timing, agreement queues, readiness conditions.

A County Office

SCCOE is the prime and program lead. A county office that wants to convene its own districts can use the same MOU structure, incentive design and readiness instruments; the executed agreements are on this site.

A Shared Implementation Journey

Four Steps A District Takes.

The same four moves the cohort's deliverables follow, as a district experiences them: Listen & Understand · Connect & Plan · Participate & Support · Reflect & Grow.

  1. 01

    Listen & Understand

    Bring The Right People Together

Identify a district lead, site champions, service partners and the people responsible for student information. In the first cohort that meant a district lead plus a stipended champion at each of three sites.

    Superintendent's Office · Student Services · Technology

  2. 02

    Connect & Plan

    Understand The Starting Point

Document current services, staffing, referral practices, school priorities and systems. The eight-indicator readiness rubric and the site assessment are the instruments this program used. Both are published here.

  3. 03

    Participate & Support

    Prepare A Dependable Support Pathway

Clarify what happens after a strength or concern surfaces in a student's own words: who responds, through which tier, and how service receipt is confirmed. Route it through the MTSS process the school already runs.

Wellness Staff · MTSS And COST Teams

  4. 04

    Reflect & Grow

    Learn From Subsequent Outcomes

Connect documented support to later attendance, engagement, academic or other agreed indicators where authorized. A change over time is evidence to study, not automatically a causal effect.

Start With What Already Exists

People. Processes. Partnerships. The readiness record helps a district see both what it can build on and where additional support is needed. In Cohort I, technology was not the constraint anywhere; schedule and staffing were. Where a screening instrument already existed, a tier structure had formed around it; where none existed, there was no referral pathway to route into. Read The Readiness Findings →

What A Cohort I District Committed To

Six Commitments, One MOU.

From the Invitation to Participate and the executed SCCOE–LEA memoranda of understanding. In return SCCOE provides $120,000 per LEA, distributed equally across at least three participating school sites, paid in four installments tied to readiness assessment, training, collaborative learning and screening administration; term July 1, 2026 – June 30, 2028.

  1. 01Three schools across the grade span the district structure allows
  2. 02Three agreements: an MOU with SCCOE; a master services agreement and the CA-NDPA with Impacter Pathway
  3. 03A district lead and a stipended champion at each site
  4. 04A fall screening window in a transition grade: 3rd, 6th or 9th
  5. 05A standing data review inside the existing MTSS process
  6. 06Quarterly collaborative learning with the other districts

Instruments You Can Use Today

The Readiness Rubric And The Site Assessment.

Eight indicators with four-level descriptors, and the site records that apply them. Both are published from Deliverable 1 so a district can run the same assessment on its own schools.

Sustainability

Service Delivery Is Reimbursable. These Districts Are Already Billing.

Behavioral health screening and assessment in schools are reimbursable under California’s CYBHI Multi-Payer School-Linked Fee Schedule. Of the six Cohort I districts, 5 are enrolled in a fee-schedule cohort and 3 have drawn reimbursement: the sustainability pathway a district steps onto when it adopts the model, and the reason the cohort was recruited for CYBHI participation or readiness.

Where To Start

Talk To The County Office That Leads The Program, Or To The Platform That Delivers It.

The Santa Clara County Office of Education’s Youth Health and Wellness department leads recruitment and convenes the cohort; Impacter Pathway runs the readiness assessment, the screener and the training. A district can begin with either. Learn from the first cohort first: Deliverable 1 is the full account of what it took.
